Post-Exertional Malaise (PEM) is a key symptom of both ME/CFS and many cases of Long Covid. It refers to a worsening of symptoms following physical, cognitive, emotional or social activity that exceeds a person’s available energy.
Unlike normal tiredness, PEM is not simply relieved by rest and can have a significant impact on daily life.
What Does PEM Feel Like?
PEM can occur after activities such as:
- Physical activity
- Concentrating or problem-solving
- Reading or screen use
- Social interactions
- Emotional stress
Symptoms may:
- Appear immediately or be delayed by hours or days
- Be disproportionate to the activity undertaken
- Lead to increased fatigue and worsening of other symptoms
- Take hours, days, weeks, or sometimes longer to recover from
Why is PEM Important?
PEM is one of the defining features of ME/CFS and is increasingly recognised in Long Covid.
Understanding your personal triggers and limits can help reduce the risk of symptom flare-ups and support better day-to-day management.
Managing PEM
Many people find it helpful to:
- Use pacing and energy management strategies
- Balance activity with rest
- Monitor activities and symptom patterns
- Avoid pushing through worsening symptoms
- Stay within their individual energy limits where possible
Current NICE guidance does not recommend Graded Exercise Therapy (GET) for people with ME/CFS.
Learning to recognise and manage post-exertional malaise is one of the most important aspects of living with ME/CFS or Long Covid. Understanding your individual limits and responding early to symptom changes can help reduce the impact of PEM and support more sustainable energy management.
