This service offers self-referral via PhysioNow for people aged 18 years or over.

Once you’ve submitted your referral, we will register you and provide you with a link to complete an initial digital self-assessment. 

Following this, we’ll send you a text either inviting you to call or with instruction on how to book in yourself. If you cannot receive texts, we’ll give you a call instead.

Once you are referred to our pain services by your GP, we will send you a text inviting you to call and book in. If you cannot receive texts, we’ll give you a call instead.

If you don’t hear from us within 3 weeks, call 01924 792 075.

Our mobile injection unit provides injections under X-ray or ultrasound guidance performed by a Consultant Anaesthetist.

The clinical team includes a Consultant, Operating Department Practitioner, Radiographer and a Recovery Practitioner.

Self-referral is not currently available for pain management support which means you need a referral from your GP.

Support with making a referral

Self-referrals are only available for our MSK rehab services. For our MSK CATS and pain services, you need a referral from your GP or other healthcare professionals.

If you want to make a self-referral but are unable to complete a self-assessment online, or if you have a question about our service, you can call us on 01924 792 075.

A family or friend can refer you to the service if you are unable to do so. Please note that they will need to provide your name, date of birth, address and NHS number.

If you need any additional support such as an interpreter (including for British Sign Language) or any accessibility requirements, please let us know when booking your appointment.
