MSK Services
Our MSK service offers self-referral for people aged 18 years or over.
Start your self-referral hereWhen we receive your referral, we will register you and provide you with a link to complete an initial digital self-assessment.
Following this, we’ll send you a text either inviting you to call or with instruction on how to book in yourself. If you cannot receive texts, we’ll give you a call instead.
Pain services
Once you are referred to our pain services by your GP, we will send you a text inviting you to call and book in. If you cannot receive texts, we’ll give you a call instead.
If you don’t hear from us within 3 weeks, call 01325 728 988.
Support with making a referral
Self-referrals are only available for our MSK services. For our pain services, you need a referral from your GP.
If you want to make a self-referral but are unable to complete a self-assessment online, or if you have a question about our service, you can call us on 01325 728 988.
A family or friend can refer you to the service if you are unable to do so. Please note that they will need to provide your name, date of birth, address and NHS number.
If you need any additional support such as an interpreter (including for British Sign Language) or any accessibility requirements, please let us know when booking your appointment.
