We work in partnership with the NHS to help their organisations extend their pathways into the local community, support primary care and provide patients with accessible and high-quality services.
Our community objectives for a positive impact

- Patient outcomes: Making services available and accessible with a focus on tackling inequalities
- Partnership: Supporting NHS partners with sustainable transformation, providing value for money services which reduce the burden of long-term ill-health
- Innovation: Enhancing digital health and wellbeing tools, ensuring safe, accessible, future proofed services and effective care
- Community integration of services: Supporting and developing links with local stakeholders, including primary and secondary care and voluntary sector organisations. Building sustainable long term community wellbeing.
