This service offers self-referral for people aged 18 years or over.

When we receive your referral we will register you and provide you with a link to complete an initial digital self-assessment.
You will then receive an email and/or a text message with our first available appointment for you. You can confirm or reject this appointment via the link included in the message.

If you want to make a self-referral but are unable to complete a self-assessment online, or if you have a question about our service, you can call us on 01325 728 988
A family or friend can refer you to the service if you are unable to do so. Please note that they will need to provide your name, date of birth, address and NHS number.
Please note. If your problem relates to a fracture, long COVID, recent operation, or women’s health please speak to your GP as it’s unlikely that this service will be able to provide the support you need.
