Each time you are seen by one of our clinicians, they record what you tell us and what is discussed. This is routine practice in healthcare. It helps ensure that important information is captured, progress is tracked, and your future care can be planned.
We also hold your personal information such as your name, address, gender, next of kin and ethnic origin.
Clinic visits, operations, tests and investigations are documented in your health record, alongside copies of all relevant communications (e.g. letters).
If you’ve had a scan in one of our diagnostic services
If you’ve had an X-ray, MRI, CT or ultrasound (US) scan that was provided by a Cora Health diagnostics team you can request a copy of your diagnostic images.
Request a copy of your diagnostic imagesThe report of your scan will have been sent to the person who referred you. In most cases this is a GP or hospital clinician. It is recommended that the outcome of a scan is discussed with you by the person who requested it.
If another healthcare provider needs access to your scan images, please ask them to contact us directly at [email protected].
If you need access to your full care records
If you need to access to all your care records held by Cora Health you can make a Subject Access Request (SAR) via our online portal.

Once complete, you will be able to view your health records using a download function within the portal. By not printing and sending paper copies we can manage requests more quickly and keep all information secure. When making your request, please ensure the correct application type is selected. This will also help our team process your request efficiently.
It is a legal requirement that we ask you to supply identification as part of the request process.
Confidentiality
We keep your care records confidential at all times. All of our team work to strict professional and contractual codes of confidentiality. Where possible, we will anonymise information so that individual patients cannot be identified.
The only time information about you is shared within our team is when it is necessary for them to carry out their job. The only time we share information with third parties is if they are directly involved in your care. For example, your GP or hospital team.
