This service offers self-referral via PhysioNow for people aged 18 years or over.

When we receive your referral from a GP, we will register you and provide you with a link to complete our digital self-assessment tool, PhysioNow.
You will then receive an email and/or a text message with our first available appointment. You can confirm or reject this appointment via the link included in the message.
Support with making a referral
If you want to make a self-referral but are unable to complete a self-assessment online, or if you have a question about our service, you can call us on 01522 305656.
A family or friend can refer you to the service if you are unable to do so. They will need to provide your name, date of birth, address and NHS number.
Please note. If your problem relates to a fracture, long COVID, recent operation, or women’s health please speak to your GP as t’s unlikely that this service will be able to provide the support you need.
