This service offers digital self referral 24 hours a day for people aged 18 years and older via PhysioNow.
Complete a digital assessment for self-referralThe service is also available for those aged 16 and 17 years via a referral from your healthcare professional. If you require further information you can contact us on 03301 244 652.
My GP has already referred me
When we receive your referral from a GP, we will register you and provide you with a link to complete our digital self-assessment tool, PhysioNow.
You will then receive an email and/or a text message with our first available appointment. You can confirm or reject this appointment via the link included in the message.
Support with making a referral
If you want to make a self-referral but are unable to complete a self-assessment online, or if you have a question about our service, you can call us on 03301 244 652.
A family or friend can refer you to the service if you are unable to do so. They will need to provide your name, date of birth, address and NHS number.
Please note. If your problem relates to a fracture, long COVID, recent operation, or women’s health please speak to your GP as t’s unlikely that this service will be able to provide the support you need.
