Refer yourself for assessment and treatment
This service offers self-referral via PhysioNow for people aged 18 years or over.
Self referral will be available from 1 April 2026.
Support with making a referral
If you want to make a self-referral but are unable to complete a self-assessment online, or if you have a question about our service, you can call us on 01268 904 102.
A family or friend can refer you to the service if you are unable to do so. They will need to provide your name, date of birth, address and NHS number.
My GP has already referred me
In some cases a GP will make a referral on your behalf. When we receive a GP referral the information provided is reviewed by one of our clinical team – this is known as clinical triage.
Once triage is complete, we’ll contact you about an appointment. This could be a text message with a link to book an appointment for yourself, a phone call to arrange an appointment, or a letter to let you know that we have referred you to another service better suited to the care you need (e.g. the hospital orthopaedic team).
