If you want to make a self-referral but are unable to complete a self-assessment online, or if you have a question about our service, you can call us on 01865 634 336.
Please note. If your problem relates to a recent fracture or operation, or women’s health, then please speak to your GP who can consider a professional referral for you.
A family or friend can refer you to the service if you are unable to do so. They will need to provide your name, date of birth, address and NHS number.
My GP has already referred me
In some cases a GP will make a referral on your behalf. When we receive a GP referral the information provided is reviewed by one of our clinical team – this is known as clinical triage. Once triage is complete, we’ll contact you to schedule an appointment. This could a text message with a link to book an appointment for yourself, a phone call to arrange an appointment, or a letter to advise you that we have passed your referral onto another specialist service (e.g. the hospital orthopaedic team).
